Adobe
Acrobat Reader Documents
To open the Adobe Acrobat version of a form, click on the link under the "Form" column.
Forms are created in this Portable Document Format (PDF) so that you can
view, print, and even fill them out on many computer platforms. These platforms
include DOS, Windows, Macintosh, OS/2, and Unix. To view and print these
files, you must have Adobe Acrobat Reader software installed on your computer.

This lets you know that if you click on the link
under the "Form" column,
you will be taken to the web site of the department that hosts the
form.
"Input-Enabled" Forms
This lets you know that if you click on the link
under the "Form" column,
you will be taken to the web site of that electronic form. You can
input data into this form, and the information will go to the appropriate
person. It is highly recommended that once you enter information into
one of these forms to print it out for your records before you submit
your information.
Microsoft
Excel Documents
To download the Microsoft Excel version of a form,
click on the link under the "Form" column. You will receive an option to open
or save the document. To use the downloadable versions of these forms,
you will need a version of Microsoft Excel. If you do not have Microsoft
Excel, you can download Microsoft Excel Viewer, which will allow you
to view and print the forms (thus making them "read-only").
You cannot, however, edit these forms using Microsoft Excel Viewer.

Microsoft
Word Documents
To download the Microsoft Word version of a form,
click on the link under the "Form" column. You will receive an option to open
or save the document. To use the downloadable versions of these forms,
you will need a version of Microsoft Word. If you do not have Microsoft
Word, you can download Microsoft Word Viewer, which will allow you
to view and print the forms (thus making them "read-only").
You cannot, however, edit these forms using Microsoft Word Viewer.
