need to be completed can now be filled out on-line. After
the form is completed, simply print it out, sign it, and
send it to the appropriate representative.

Request For Quotation Form Buyers use this document to request quotes from vendors and now you can

Vendor Evaluation Form You can now let us know of any positive or negative experiences you have
had with our vendors. Fill out this form to give us feedback.
Materials/Services Contract This is a downloadable contract that can be customized for campus projects
requiring a written agreement that specifies license, insurance, and
indemnification requirements. This form is now able to be filled out
on-line.

Directors This is a file that contains contact information for the Buildings and
Grounds directors of the Maricopa Community Colleges.
Personalized
Letterhead Template This is a template to "Personalize" the "Generic Letterhead",
which has no formatting. This template allows you to personalize the letterhead
for your office or department.

Employment Form The Special Services Employment Form is on the Legal Department web site.

Certificate
of Insurance Database Before a PO can be issued or a contract
is signed, a valid certificate of insurance is required
from the vendors/contractors that are doing business with
MCCCD. The link above is a "searchable" database
for all certificates of insurance on file from vendors
and contractors.

Services Contract Click on the link above to go to the MCCCD Legal Department
web site to find instructions on (and also download) the
Professional Services Contract.
Non-Profit Organzation Letter MCCCD is not a non-profit organization. Section 501 (c) (3) of the Internal Revenue Code does not apply to MCCCD. Please see the above official letter regarding this.
