Steps for Participation
- Review the ‘Skills Matrix’ and identify your desired learning outcomes.
Visit the Community Partners page to select a location for your community based learning.
If you wish to participate in community based learning with an agency that is not on the list, please contact the Creative Pathways office.
- Download the Maricopa Learns Interest Form. Return the completed form to the
Creative Pathways office. Please contact Creative Pathways
if you have questions.
- Meet with your Supervisor/Chair to discuss your placement idea and obtain support for your participation. At this point
you will need signatures from your Supervisor/Chair and Director/Vice President. Now you’re ready to submit your Interest
Form to the Creative Pathways Coordinator.
- The Creative Pathways Coordinator will confirm that your project plan meets the criteria and will contact the Community Partner to complete Site Approval process. Process includes Site Safety and Health assessment, review of employee’s learning objectives, and signature of the Community Partner Agreement.
- Once this is done, you will receive notice that your plan has been approved. Now you can complete the Employee Acknowledgement form, your Placement Agreement and get all the necessary signatures including that of your Placement Supervisor and those of your Maricopa Supervisor/Chair, Director/Vice President, and Vice Chancellor/President.
- You will then be asked to participate in a brief orientation session describing community based learning.
- Now it’s time to begin your placement. You will be required to submit a Time Log to the Creative Pathways Coordinator at the end of each month during your placement.
- At the end of your placement, you will be asked to complete an Evaluation of the program
- Your Community Partner will also be asked to complete an Evaluation of the program.
