Complaint Form
for Employees
One
of the functions of the MCCCD EEO/AA Office is to investigate complaints
of discrimination based on race, color, religion, national origin, sex
(including sexual harassment and pregnancy), sexual orientation, age (over
40), disability, or Vietnam-era veteran status. Under District procedures,
employee complaints are investigated directly by the District EEO/AA office.
The formal investigatory process is begun by completing and signing this
Discrimination Complaint Form. Formal complaints must be filed within
180 days of the most recent alleged act of discrimination. All files
and documentation relative to the investigation will be maintained in
the EEO/AA Office. If you have questions about the investigation procedure
or any other concerns with regard to filing a complaint, please call 480-731-8885.
To prove unlawful or District-prohibited discrimination, it is necessary
to show that under similar circumstances, you have been treated differently
(less favorably) than other individuals because of your race, color, religion,
national origin, sex, sexual orientation, age, disability, or Vietnam-era
veteran status; or, that you have been retaliated against for either filing
a discrimination complaint or participating in a discrimination complaint
investigation.
If you are an employee and feel you have been treated unfairly but the
unfair treatment is not based on your race, color, religion, national
origin, sex, sexual orientation, age, disability, or Vietnam-era veteran
status, submitting a formal complaint on this form is likely not the appropriate
or most effective means of addressing your concern. Contact the

Relations Department for possible resolution.
