- Open Enrollment is an annual event that generally takes place in April, during which benefit-eligible employees can review their benefits and make changes that take effect July 1.
- Changes are made in HRMS-Self Service.
- Not changing your benefits - no need to do anything. Current elections will roll to the new fiscal year.
- After Open Enrollment ends any changes made to your benefit elections must be due to a "Change in Family Status".

Supplemental Life

If you are electing Supplement Life for the first time or wish to increase your current coverage during Open Enrollment - Evidence of Insurability is required for any amount added or increased.
Dependent Life Insurance
There are no plan changes this year. Premium rates did not change.
If you are electing Dependent Life for the first time or wish to increase your current coverage during Open Enrollment - Evidence of Insurability is required for any amount added or increased.
The amount of Dependent Term Life Insurance will be reduced by 50% on your spouse's 70th birthday.
Dependent Life coverage will not cover dependents active in the military.
Accidental Death and Personal Loss

Short-Term Disability
There are no plan changes this year. Premium rates did not change this year.
Electing Short-Term Disability for the first time or increasing the benefit level more than one step - Evidence of Insurability is required.
You may increase coverage by one benefit level, as your salary allows, without evidence of insurability.
Employee's currently on a Waiver
All Waivers must be re-submitted each fiscal year. Failure to provide a new waiver each Fiscal Year will result in the loss of the waiver. Should this occcur the employee will be enrolled in Employee Only Core medical coverage effective July 1st of the new Fiscal Year.
Please keep a copy of the completed waiver for your records before sending it to the Benefit Office. When the waiver arrives in the Benefit Office, a written confirmation verifying receipt will be sent to the employee. If confirmation is not received by May 1, 2012, please call the Benefits Office at (480) 731-8415. May 4th is the deadline to submit a Medical Waiver for FY 2012-13.

After completing your medical changes in HRMS, print the waiver form from the Medical Waiver link at the top of the Enrollment Summary screen and send it to the Benefits Office. May 4th is the deadline to submit a Medical Waiver for FY 2012-13.
