- If you receive covered services outside of Arizona from a First Health PPO provider, benefits are paid at the in-network level.
- Services received outside of Arizona from a provider who does not participate as a First Health PPO provider, are paid at the out-of-network level.
- Pre-certification requirements and other benefit plan limitations apply to services received outside Arizona.

- It is recommended that you locate the providers in the area prior to traveling.
- The First Health network of providers is not available for travel outside of the United States.
- The Core plan does not have out of network benefits. If outside of Arizona, utilize the First Health network.
- All services rendered outside the United States should be submitted to Zenith Administrators at 2001 W. Camelback Rd., Suite B350, Phoenix, AZ 85015.
- Charges submitted in foreign currencies will be converted to U.S. Dollars at the "date of service" applicable Markets Currency Exchange Rates when paid for with a major credit card.
- Only emergency services are reimbursed at the in-network level.
- Non-emergency services are covered at the out-of-network level.
- Non-emergency, out of country medical expenses are not covered for those on the Core plan.
- The Maricopa Community Colleges also offers travel assistance benefits provided by Aetna Travel Assistance Services for benefit eligible employees.
