- Contact Zenith at 602-248-8434 if you have trouble logging in.
- View personal eligibility and paid claims information.
- Check current claim status.
- Link to any of the other service providers, such as PPO providers, utilization review or pharmacy.
- You can also download plan documents and claim forms.
Remember: BC/BS is used for their physician, hospital, lab/x-ray and urgent care provider directories only. Zenith provides all claims/service support and handles all billing issues.
