Describe the activity you are requesting, including the desired learning outcomes, description of audience (e.g., PSA, managers, mixed, etc.), approximate number of attendees, etc.

Provide a primary and back up date/timeframe and approximate times for activity to be conducted, e.g., February 2009 in the morning between 8 and 10:00 am, etc.

Provide the location/building/room that would be used for the activcity, including the options for room set up, e.g., theater style only, or room can be configured in any style necessary.
