Women’s Health and Cancer Rights Act

The Women’s Health and Cancer Rights Act of 1998 requires group health plans and health insurance issuers that cover mastectomies to also cover re-constructive surgery or other related services following a mastectomy.

MCCCD offers the choice of two PPO plans. 

Plan Choices

  • Core Plan
  • Buy-Up Plan

Provider Alternatives

Your out-of-pocket costs will differ depending on which type of provider you select.

In-Network Providers (PPO) - These providers have agreed to accept a negotiated fee for covered services.  Your co-pays are lowest when you use PPO providers.   

Out-of-Network Providers (Non-PPO) - Arizona health care providers who are a part of the participating provider network.  These providers have agreed to accept the allowance for covered services, and will file claims for you.  Deductibles and coinsurance will be charged when using these providers.

Non-Contracted Providers (Non-PPO) These providers do not have an agreement with the PPO network.  These providers may charge the difference between their billed charges and the allowed amount. Deductibles and coinsurance will be charged when using these providers.

You are not required to have a primary care physician direct your care.  You do not need a referral to see a specialist or other provider.  You choose whether to use an in-network or out-of-network provider.
