What Happens to My Insurance When I Terminate?

 

If your employment terminates, for whatever reason, you need to know what happens to your benefits.

Medical and Dental Insurance
The Medical and Dental plans end on the last day of the month in which you terminate your employment. For example, if you terminate August 15, your medical insurance will end on August 31.

COBRA
The Consolidated Omnibus Budget Reconciliation Act of 1985 (COBRA) is a federal law that allows you to continue medical and/or dental coverage at your own cost for up to 18 months. Following your termination, COBRA enrollment materials will be sent to your last known home address. Update your address with the District Office prior to your employment end date. Send an email to payroll@domail.maricopa.edu or call the Compensation Department.

Disability Insurance
Disability coverage ends on your last day of employment.  If you are receiving Disability benefits when you terminate employment, your claim MAY remain open as long as you continue to meet the disability requirements.

Term Life Insurance
Basic Life, Supplemental Life, and Dependent Life Insurance ends on the last day of the month in which your employment terminates. Accidental Death and Personal Loss coverage ends on the last day of the month in which your employment terminates.

At the time of your termination, if you had a basic life insurance policy with a face value of $20,000, you may be eligible to convert a portion of that amount to a personal policy. If you had a supplemental life insurance policy, you may be eligible to port or convert this coverage to a personal policy. These changes must be accomplished within 31 days of the last day of the month in which you terminate. Please call Aetna at 1-800-826-7448, and they will provide you with the information and forms to make these changes.

Life Insurance, Accidental Death and Personal Loss Coverage may be continued past the maximum age for your fully disabled dependent child. However, such coverage may not be continued if the child has been issued a personal life conversion policy. Proof that your child is fully disabled must be submitted to Aetna no later than 31 days after the date your child reaches the maximum age. To obtain an application contact the Compensation Department at 480-731-8415.

Arizona State Retirement System
You may continue to invest your contributions with Arizona State Retirement or you may apply for a refund by contacting the Arizona State Retirement Fund. If you have any questions about your state retirement funds or wish to withdraw contributions, please call the Arizona State Retirement Fund at 602-240-2000.

Tax Shelter Annuities (TSAs)
If you have contributed to a TSA these deductions will stop. If your employment should restart, you will need to complete a new TSA deduction form with your TSA agent. 

Health Care Flexible Spending Account
If you have a balance in your account after you terminate, participants have 90 days to submit reimbursement requests for eligible expenses. Expenses must have been incurred while you were an active participant in the plan. 

Dependent Care Flexible Spending Account
If you have a balance in your account after you terminate, participants have 90 days to submit reimbursement requests for eligible expenses. Expenses must have been incurred while you were an active participant in the plan.
