New Benefits Information Effective July 1, 2008


What's New FY2008-09

Documentation needed for all Insurance changes!

Documentation must be provided if you wish to add a dependent (spouse, child, or
partner) to your health coverage. Health coverage affected includes medical, dental, dependent life and family accidental death and personal loss insurance.  See 'What's New FY2008-09' for specific details.

Arizona State Retirement System (ASRS) -

ASRS has increased the contribution rate for FY08-09.  The new contribution rate will be 9.45%.  This will include 8.95% for pension contributions and .50% for LTD contributions.

Disabled Dependent Children Covered under Life and AD&PL Insurance -

Dependent Life Insurance and Accidental Death and Personal Loss Insurance for your fully disabled dependent child may be continued past the maximum age for dependent children.  However, such coverage may not be continued if the child has been issued a personal life conversion policy. 

Proof that your child is fully disabled must be submitted to Aetna no later than 31 days after the date your child reaches the maximum age.  Please see the Life and AD&PL sections for detailed information.

Faculty 12- Month Pay Option -

Faculty may choose to be paid over a 12-month period from July to June or a 9-month period from August to May.  Faculty who wish to stay in their current pay cycle do not need to do anything. 

 

Faculty who wish to change to the 12-month pay cycle must complete the Faculty Pay Cycle Enrollment Form.  Faculty who are currently on the 12-month pay cycle and wish to return to the 9-month pay cycle must complete the Faculty Pay Cycle Enrollment Form. 

 

All 9-month Faculty, regardless of their pay cycle, will receive the Faculty Pay Cycle Enrollment Form in their open enrollment packet.  After signing the form send ALL copies to the Benefits office.  The employee copy will be returned to the employee as confirmation that is was received and approved by the Benefits office.  All forms must be received in the Benefits office by April 30, 2008.

All Faculty, whether on a 9-month or 12-month pay cycle will accrue their sick leave in 9-months from August through May.  This will allow you to accrue your full yearly sick leave accruals during your regular 9-month assignment. 

Enrollment Elections -

If you do not make any changes, all current coverage elections will stay the same for FY08-09.  Open Enrollment is from April 1 - April 30, 2008.  After Open Enrollment ends any changes made to your benefit elections must be due to a "Change in Family Status".
