Zenith Administrators - Medical Administrator

Zenith processes claims, generates Explanation of Benefits (EOB) statements, provides customer service and issues ID cards for medical services. 

Blue Cross/Blue Shield of Arizona is used for their provider network for Arizona residents only.  Claims should not be sent to BC/BS.  Claims received by BC/BS will be denied.

For 24 hour access to benefit plan information go to www.zenithadm.com.  Employee's must log in to view MCCCD's PPO plan.  Zenith provides the log in information to employees.  If you have lost or do not remember your log in information contact Zenith at 602/248-8434

The Zenith web site allows you the ability to view personal eligibility and paid claims information, check current claim status and link to any of the other services providers, such as PPO providers, Utilization Review or Pharmacy.  You can also download plan documents and claim forms.

Remember:  BC/BS is used for their physician, hospital, lab/x-ray and urgent care provider directories only.  Zenith provides all claims/service support and handles all billing issues. 

Medical Provider Search - scroll down and choose "Not a Member Yet" and then the "PPO" option.

Urgent Care Search - scroll down and choose "Not a Member Yet" and then the "PPO" option.  Then choose Urgent Care Centers.
