Forms and Resources

International Travel Forms:

International Travel Policy – Adopted by the Maricopa Community Colleges Governing Board February 27, 2007, Motion No. 9414.
 
International Travel Proposal – This form has the international travel request embedded into it. Everyone in the District who wishes to travel internationally must complete this form and receive the necessary approvals as indicated on the form. It must be submitted along with a three-part Travel Authorization Form. Instructions are included. Faculty leading education abroad programs fill out an education abroad program proposal in place of this form.
 
International Travel Report – All who travel internationally, including faculty leading student education abroad programs are required to report on their travel outcomes by completing this one page form. Instructions are included.   This becomes part of the quarterly international travel report that is submitted to the MCCCD Governing Board. Faculty leading education abroad programs also complete an on-line evaluation (see below).
 
Status Report on International Travel Outcomes – All faculty who participate in faculty development programs and faculty exchange programs are required to report on the status of travel outcomes within one year following their return to the United States.

Students Forms:

Student On-line Evaluation of Education Abroad Program – This form contains instructions and a link to the student on-line evaluation of education abroad programs.
 

Education Abroad Faculty Forms:

Education Abroad Program Proposal – For use by faculty submitting proposals for approval to lead education abroad programs. Proposals are due by the Week of Accountability. Please note: If this is a first time education abroad program, you will need to complete a site assessment proposal.
 
Multi-Year Renewal Form for Education Abroad - For use in renewal of previously approved programs. 
 
Faculty Course Information FormMay be used by faculty who are submitting an education abroad proposal for a program with several participating faculty. In this case, only the faculty program director would need to submit the full proposal. The participating faculty need only submit their specific course information (including contact hours) as an attachment to the main proposal.
 
Site Assessment Proposal – New Education Abroad ProgramThis is a proposal to visit a proposed new education abroad site. (This takes the place of the International Travel Proposal and has an international travel request embedded into it.)
 
Site Assessment Report – Upon completion of the site visit, the faculty member is required to submit a detailed report about the findings and appropriateness of the site for a future education abroad program with students. The faculty member is also required to file the International Travel Report as per District policy. 
 
Faculty On-line Evaluation of Education Abroad Program – This document outlines the simple steps to take for faculty leading education abroad programs to self evaluate the class they taught abroad.

Education Abroad Crisis Management Plan– A handbook for Maricopa faculty leading education abroad with detailed information on the district's policy for responding to emergencies abroad.

Other Forms: 

International and Intercultural Education Innovation Grants (Internal/Mini-Grants) – Application and Guidelines for internal grant funds that support MCCCD projects and programs with an international/intercultural education focus. Grants are administered by the District Office of International and Intercultural Education; proposals are reviewed and voted on by the IIE Steering Committee.
