1. Complete the online CITI Program Training for the Protection of Human Subjects in Research. (Be sure to enroll under "Maricopa Community Colleges" at: http://www.citiprogram.org/ . There is no charge to Maricopa Employees and Students.)
2. Complete the Application to Conduct Human Subjects Research. http://www.maricopa.edu/irb
Application Form (editable MS Word form)Both you and the investigator from the other Institution must complete the application as Principal Investigators (PI) or as Co-PIs.
3. Provide Institutional Review Board Approval from the Partner Institution or indicate submission status if seeking concurrent approval.
4. Submit the following documents by "mail" and email:
Include the appropriate signatures on the hard copy.
Be sure to include all relevant materials. Pay special attention to information that applies to your proposed research (such as informed consent, data collection instruments, special populations, institutional IRB approval).
5. The IRB Office will contact the applicant if additional information is needed.
6. The IRB Office will subsequently notify the applicant of the decision.
7. Please allow ample lead time in submitting your application.
