1. Complete the online CITI Program Training for the Protection of Human Subjects in Research. (Be sure to enroll under "Maricopa Community Colleges" at: http://www.citiprogram.org/. There is no charge to Maricopa Employees and Students.)
2. Complete the Application to Conduct Human Subjects Research. http://www.maricopa.edu/irb
3. If you seek to conduct research in the Native American community, be sure to provide approval from that community. (If you are surveying all ethnic groups on campus, including Native American, then you should not need such approval.)
If you plan to do research at one of the colleges, be sure to get the approval of the Vice President of Academic Affairs (VPAA) at that college.
4. Submit the following documents, by "mail" and mail:
Include all relevant information. Pay special attention to areas such as informed consent, benefits and risks, confidentiality and protection of information, conducting research with those under age 18).
5. The IRB Office will contact the applicant if additional information is needed.
6. The IRB Office will subsequently notify the applicant of the decision.
7. Please allow ample lead time in submitting your application.
