Maricopa
Provides
Student Insurance Coverage
On
January 25, 2000, the Governing Board awarded the contract for student
accident insurance coverages effective February 1, 2000 to the Renaissance
Agencies of Santa Monica, California. These coverages consist of student
accident insurance covering all enrolled students (including athletic
injuries), special risk accident and sickness insurance for trips, and
voluntary student health insurance. (The catastrophe and supercatastrophe
coverages are still in effect until July 31, 2000 with Student Insurance
of Los Angeles, California.)
When
a Maricopa student is injured while participating in or attending an activity
exclusively sponsored, calendared, or supervised by the college, the student
should first submit their claim to their primary accident or health insurance
provider. (This includes any other insurance that he/she has in force,
such as any individual or family insurance policy.) If the student has
no other accident or health insurance in force and/or has exhausted the
limits of any other insurance policies, then the student accident insurance
policy becomes primary and will pay all eligible expenses according to
the provisions of the Master Policy.
Renaissance Agencies is providing the identical coverages as those offered
previously by Student Insurance, except the voluntary student health insurance
policy contains several enhancements. Here's a brief summary of our student
accident insurance plan. (Please refer to the Master Policy for complete
details of coverages, limitations, and exclusions. The Master Policy will
prevail at all times.)
Each person registered as a day or evening division student who is enrolled
in an approved credit and/or noncredit class is eligible for the plan.
The
plan covers accidents occurring while the student is:

Attending class during the hours when college is in session.
- Traveling
for regular class sessions, directly to or from home and college other
than by motorized vehicle, but not longer than one hour before class
commences or one hour after class is dismissed or for the time required
when traveling in an authorized and furnished school vehicle.
- No
coverage is provided for any motorized vehicle not supervised and provided
by the District.
- Participating
in or attending an activity exclusively sponsored,
calendared, and supervised by the college; and traveling to and from
the activity in an authorized vehicle.
Coverage is provided for the children of properly enrolled and registered
students and staff members of the college while they are authorized to
use, and only while they are using, the officially designated childcare
facility on or off campus.
The maximum benefit payable is $25,000.
If commencing within 120 days after an accident, the policy will pay all
eligible expenses which:

Are incurred within one year after the date of the accident.
- Are
not covered under any other insurance accident or health insurance plan.
Claim forms must be completed and sent to Personal Insurance Administrators,
Inc. of Thousand Oaks, California within 120 days after an accident.
Accidental Death and Dismemberment benefits (see Master Policy schedule)
are also provided.
