Securing Nutrition Assistance

The Supplemental Nutrition Assistance Program (SNAP) is a monthly nutrition program that provides funds for qualified individuals and families to purchase healthy food from local grocery stores, farmers markets, or online.

SNAP gives you and your family the money you need to buy nutritious food so you can stay healthy and thrive. The Supplemental Nutrition Assistance Program helps households pay for food, freeing up money spent on groceries to pay for other essential items, like rent or college tuition. Benefits are electronically transferred to a card each month that you can use to pay for groceries.

Required Documents

You may need to provide proof for all the items listed below that apply to you to get the maximum benefit available to you, but your application will not be processed without verification of your identity, address, Social Security numbers, citizenship status, and income. Some pieces of information may be verified through electronic data

Proof of Housing Costs
  • Wage Stubs and Statements of Tips
  • Letter on Letterhead From Employer
  • Benefit Award Letters
  • Checks, Other Statements, etc.
Proof of Utility Costs
  • Fuel Bills/Shut-off Notice
  • Non-heating Utility Bills
  • Telephone Bills, etc.

 Proof of Medical Expenses

  • Medical Bills From Hospitals or Clinics
  • Pharmacy Printouts
  • Health Insurance Premium Receipt or Stub
  • Payment Agreement with Doctor and/or Hospital, etc.

Proof of Disability, if applicable

  • Disability Benefits Award Letter
  • Statement From a Medical Professional

Proof of Court-Order Child Support Payments

  • Court Order and Proof That You Are Making Payments

Proof of Relationship/ Household Composition

  • Birth Certificates
  • Marriage Licenses
  • Insurance Policies, Wills, etc.

Proof of Dependent Care Costs

  • Child Care or Day Care Receipts
  • Statement From Provider

Proof of Resources

  • Bank Statements Sale or Transfer Papers for Property/Deeds

Proof of Enrollment at a Higher Education Institution/FWS Program

  • School Schedule
  • FWS Agreement 

 

Students will be required to submit proof of enrollment:

  • School Schedule **can be accessed in MyMaricopa Portal**
  • Proof of Federal Work Study sheet **DES will provide student with worksheet to be signed by student and a Maricopa Community Colleges employee or work-study supervisor**

Application Process

Step 1: Apply

Fill out as much of the application as you can. To submit it, you must enter your name and address (if you have one), and sign the form.

  • Online: Begin the application at www.healthearizonaplus.gov/Login/Default
  • In Person: You can find the information for your local county office at www.azccrr.com/health-and-social-services.html
  • Mail/Fax: You can download an application here
    After printing and filling out an application, you can mail, fax, or hand deliver it to your local county office. You may also request an application by phone, in person, by fax, or by mail from your county office.

Step 2: Get Interviewed

Phone or in-person interviews are required. Do not miss your interview! Your SNAP worker will go over your application and review all the documents you have available. Interviews take place anywhere from the day of submission to 30 days after. Many take place within five days of submitting. You will be contacted by phone to schedule an appointment, so be sure to answer unknown numbers after submitting your application.

Step 3: Submit Missing Documents

Submit documentation to verify all the information you provide on your application as early in the application process as you can. After your interview, you will be notified if required documents are missing and given 10 days to provide them. Your application will be rejected if certain information is not verified. Step 4: Watch and Wait

If you fulfill all application requirements, you will find out whether or not you are eligible for SNAP benefits within 30 days of the date your signed application was received.
